Application Process:

 

All services require OPWDD eligibility. Application for services can be made through one of the following:
 

• Referred to Holy Childhood for supported employment or Community Based Work Assessment through ACCES-VR (requires established eligibility for ACCES-VR services).
   

• Complete application for OPWDD funded supported employment for students age 18-21.  Must be eligible for Medicaid Waiver services
   

• Complete application for OPWDD Employment Training Program for individuals 21 and older.  Must have Medicaid services and be eligible for Medicaid Waiver services.
